TeleLeaf provides access to licensed doctors for the sole purpose of obtaining a legal Medical Marijuana recommendation in qualifying states. Our doctors and staff will not review or sign any outside documents on behalf of the patient for employment purposes or any other reason. This includes but is not limited to employment agreements, ADA documentation, and others.
Although it is not required, we’re happy to provide patients with a copy of their recommendation once it has been processed. Records must be requested by the patient via email at firstname.lastname@example.org. Our support team will contact you to verify your identity and email address before releasing any information.
TeleLeaf will not release patient data to anyone other than the patient and/or registered caretaker. Additionally, TeleLeaf will not provide patient information to any outside parties other than the patients and/or caretakers themselves.
